Quality of Life Questionnaire "Qualeffo-41"
This questionnaire, first published in 1997, is a tool used by health professionals to assess quality of life in osteoporosis patients (in the context of a survey or clinical study). The questionnaire is available here as a PDF in English, French, German, Italian and Spanish. For other languages, please contact the IOF secretariat.
